Weekly Cash Balance Report

Our boss requires us to prepare this report every Saturday but the cut-off of the data is Friday.  This is very useful especially if you have a lot of bank accounts that needs to be monitored.

R-3 Contribution Collection List

This form is used by employers who only has minimal number of employees.  This report is submitted quarterly together with the proof of payments of the SS contributions of the past 3 months.  Big companies use the diskette system prepared every month.

R1-A Employment Report

This form is used by employers to report new employees who already have a Social Security number.  For those who don't have SS number yet, the employer can still use this one but the employees need to fill up an E-1 together with their birth certificate before submitting this form.
